Community of Practice Retreat RSVP

Please complete this form so we know who to expect and can prepare for the day. We will meet over zoom or in person at Bread of Life Center, 1260 Fulton Ave. Suite A, Sacramento, CA 95825. We will begin at 12 noon (Pacific) and end at 4:30 PM (Pacific).
Those meeting in person are encouraged to stay for dinner at Famous Kabob restaurant which is next door to the Bread of Life office. Please indicate if you will stay for dinner so we can make a reservation. Bring cash to pay for your meal- roughly $25 depending on what you order (Menu here).
