The following questions are designed to assist us in getting to know your background, experience, desire for this journey, and expectations as they relate to this program. This form, along with one letter of recommendation from someone who knows your spiritual journey, is what we use to discern.
The completed letter of recommendation can be emailed to office@breadoflife.org or mailed to: Bread of Life, 1260 Fulton Ave., Ste A, Sacramento, CA 95825.
After submitting the application form below, you will be prompted to pay a $50 application fee. After we receive payment and your completed application you will be contacted for an interview. Thank you!
